com_zimbra_samba not working

Is anyone out there using the Samba Change Password zimlet? I've been using it for over a year without issue, but recently after an upgrade it stopped working. I've redeployed it, which didn't seem to help. The zimlet shows up in the UI, but when I click on it the little box doesn't pop up to enter a password. I also don't see anything about this zimlet in mailbox.log. Any tips or ideas about where I should look next?
