In the case I described, it turns out the user was running Outlook 2007 SP1. Some kind of MS Office problem is preventing him from upgrading to SP2, so I'm putting that one on the backburner until he gets that resolved.
However at my company, we've managed to reproduce an instance of the more general case:
Bug 47645 - Meeting lost from organizer's calendar when adding new attendee to exception. Currently assigned and targeted for 6.0.8. I'm hoping that the fix to this bug will clean up other issues.
