Default Can't create new user accounts

Well, I don't know why my last topic was deleted..
I'll try again:

I'm using the Open Source version of Zimbra (5.0.11) in a Centos 5 (64bit).
I can't create new accounts for 2 versions now, 5.0.10 and 5.0.11 (at 5.0.9 it was working fine).

The error that I get is:

I already tried, firefox, safari, IE 6/7.. none of them work!

Thanks for anyone that help!

How odd...I can't see the image here at work on my PC running FF, but I could see your image from home last night on my Mac running FF.

Do you have any idea why this is happening or any file I could search to identify the error?

Here's the direct link for the image: http://i35.tinypic.com/161gxfr.jpg

fabioneves, do you have CLI access? If so try creating it with zmprov and see if you have better luck. Also, if you do have CLI access, I highly recommend watching the logs while you try to create the account.

What do the log files show when you attempt to create an account?
