Default Adding Users Error

When i try to add a user via Zimbra GUI i get this error message,

Message: system failure: createAccount
Error code: service.FAILURE
Method: ZmCsfeCommand.prototype.invoke
Details:soap:Receiver

When i try it via command line i get the same error but different syntax,

ERROR: service.FAILURE (system failure: createAccount)

Can anyone please help me add users, currently i have over 150 users added which i did by making a bash script.

zmprov ca email@domain.ca password

Is this a new install or an upgrade? Is zimbra running or not? What does a 'zmcontrol status' show? Is that a modified kernel you're running?

Do you have a correct /etc/hosts file and DNS A & MX records pointing to your server? If you're behind a firewall and/or router you'll need a split DNS set-up, do you have one?

This is a new Install,not an upgrade. zimbra is running just fine, I checked my /etc/hosts file,
127.0.0.1 localhost.localdomain localhost
142.25.97.33 jesseb.ca
142.25.97.33 mail.jesseb.ca

hostname of server is mail.jesseb.ca
mx records are pointing in the right direction, i do no have a split DNS setup i don't think. If you need to configure it manually i havent.
also
zmcontrol status shows everything running for mail.jesseb.ca

The hosts file is incorrect, it should be as follows:

Code:
127.0.0.1 localhost.localdomain localhost
142.25.97.33 mail.jesseb.ca mail
You shouldn't need this line:
Code:
142.25.97.33 jesseb.ca
in the hosts file at all. If you do need the entry jesseb.ca then tag it onto the second line in the hosts file.

Still same issue after editing /etc/hosts

Not sure why its not allowing me to add users for i did add 150 users before ...
