Default Spam emails still in inbox

I've been seeing a wierd occurrence, that I haven't been able to pin down. It seems that sometimes when I receive an email that the system marks as spam, it puts an unread copy in my Junk folder, however, sometimes a copy marked read is put in my inbox.

Has anyone seen this occurrence? Is there a fix for it?
