Checkboxes gone for one user

On one of my users' accounts the "Selection" checkboxes have vanished from her AJAX interface. They show up if switched to the "Standard HTML" view though.
This is the case whether or not the "Display Checkboxes" option is set in her preferences.
I have tried loading her account on different browsers and different machines with the same issue. I have tried resetting all her settings back to COS just to make sure.

Anyone had this happen or have any suggestions?
