[SOLVED] Documents Display missing?

When I view my Documents tab on my production system I see my pages as a bullet list instead of a nice formatted table with Document name, Actions, Modified by, etc etc

Any idea why this happened and how I can get back to the regular document view?
I dont see any errors in the logs, but I might not be looking in the right places.

I can still see all the pages fine, but I can see verions or modified information.
