Is it possible to store selfbuilt forms (based on the calendar form) and analyze its additional information?

Not sure I understand the question. Do you want to have your own appointment creation form? If so what types of fields do you want to add?

You're right. I want to modify the existing form by some lines with radio buttons and checkboxes, e.g.:
interesting [not at all, ... , ... , ... , complete agreement]
felt good [not at all, ... , ... , ... , complete agreement]
felt overtaxed [not at all, ... , ... , ... , complete agreement]
felt not challenged enough [not at all, ... , ... , ... , complete agreement]

For this you can try adding a Zimlet, or you'd have to modify the code directly.
