Default Documents lose CSS formatting

Hi,

On a few accounts the CSS for the Documents tab doesn't render properly. There's no CSS applied to the part of the page in which the Table of Contents or the Document goes. (See attached screen shot.)

This only appears to affect recently created accounts, I believe since 5.0.11. I don't have this problem on any accounts created before 5.0.11. I have tested in multiple browsers on both Mac and Windows and the problem is the same across the board. I just updated my server to 5.0.12 last night and the problem persists.

Does anyone have any ideas?
