Thats the typical 'can't connect to mailboxd service'. Scoll up in your log - do all the services report starting correctly (several sections up from document init) it might just have been an error somewhere after that particular startup. Most of the key stuff is done via LDAP (the -l's you see on the zmprov commands). You can email me your log (or post it here after scrutinizing it for security) if you want and I'll see if anything stands out.
