interesting, i was looking at the code for that and we check for an empty auth token before issuing the file upload request. if there is no auth token, we re-auth.
can you do the following?
1) check the profile (control panel mail applet) to make sure that all the info in there is correct for this user
2) restart outlook
3) send/receive and check to see if they are getting new mail, appts, etc...
4) grab some more logs around this file upload exception and get them to us.
please enable http logging and verbose logging in the logging control applet.
you can do this by posting them, private messaging me or opening a support incident.
i'm still hunting for a bug that describes this situation. if you open a support incident, they can file a bug for you.
