 imapsync is a good bet
I'd suggest first setting up the other server (if you want Open-Xchange, installation is nontrivial, so search for the most detailed walkthroughs you can find) and then using imapsync to get your mail data migrated. You can export your address books as CSV or vCard, so pick whichever format your new server supports.
