Was there ever a resolution to this issue? We are seeing the same thing in our environment. I do not know if it is simply looking for something that is not in LDAP and the fact that it is not there is perfectly acceptable (thus just a NOTICE and not a WARNING or ERROR) or if it reflects a misconfiguration (LDAP_OPERATIONS_ERROR). Everything seems to be working fine. Our logs are clean otherwise. We took a packet trace but we are doing start_tls so the data is encrypted. Before we start disabling encryption to troubleshoot further, we thought we'd ask the list if there was a quick answer to what this message is saying? Is there?
