Second try In the meantime I uninstalled everything (except for Windows Server itself) and reinstalled. I figured that I forgot to install SP1 for Outlook 2007 the first time. The fresh install has everything as indicated in the manual now including all Outlook updates. But the problem remains!
In the manual there is a section "Verify that the user is properly initialized in the BlackBerry Agent". It says to look for the e-mail address in dht _MAGT_ logs of BB. I don't have any entries for users in there!
Shutting down and restarting BB Agent did not help...
